About The Position

PURPOSE OF POSITION The Prevention Navigator – Health Navigation Services provides comprehensive navigation and support services to individuals at risk for HIV infection or those seeking HIV prevention resources. The Prevention Navigator works to engage clients, assess prevention needs, and assist individuals in accessing HIV testing, PrEP services, STI screening, and other health and social services. Through individualized navigation and follow-up, the Prevention Navigator helps reduce barriers to care and ensures clients are connected to appropriate prevention, medical, and supportive services. The position plays a key role in strengthening prevention efforts and improving health outcomes in communities disproportionately impacted by HIV. The Prevention Navigator ensures services are delivered in accordance with NYS AIDS Institute program standards, confidentiality requirements, and agency policies.

Requirements

Associate’s in public health, Social Work, Human Services, or related field (bachelor’s degree preferred).
At least 1–2 years of experience in community health, HIV prevention, outreach, or health navigation services.
Knowledge of HIV prevention strategies, sexual health, and harm reduction approaches.
Strong communication, engagement, and client support skills.
Demonstrated experience working with immigrant and LGBTQ+ communities.
Knowledge of HIV prevention strategies, PrEP, and Ryan White services.
Strong interpersonal and engagement skills.
Ability to work evenings and weekends as needed.
Minimum one (1) year of similar work experience, facilitating groups and performing Outreach/community engagement activities.
Excellent writing and communication skills; including multi-media outlets.
Work experience with LGBTQ individuals in a medical/health or social service settings
Knowledge of citywide resources
Experience in community based medical/health or social service settings, social work, counseling, nursing, public health or other health education.
Must pass background check

Responsibilities

Provide individualized health navigation services to individuals seeking HIV prevention resources.
Conduct client intake and prevention needs assessments to identify barriers to accessing prevention and health services.
Assist clients with linkage to HIV testing, STI screening, PrEP services, and other prevention interventions.
Provide assistance with scheduling appointments, completing referrals, and navigating healthcare systems.
Conduct follow-up with clients to support retention in prevention services and ensure successful service linkage.
Assist clients with insurance enrollment, benefits navigation, and access to supportive services when appropriate.
Conduct outreach activities to inform community members about HIV prevention services and available resources.
Participate in community outreach events, health fairs, and prevention initiatives.
Collaborate with community organizations and healthcare providers to strengthen referral networks and service access.
Utilize outreach strategies including community engagement, partnerships, and social media to reach individuals who may benefit from prevention services.
Provide education and counseling on HIV prevention strategies, including PrEP, PEP, condom use, and harm reduction.
Support clients in developing personalized prevention plans.
Provide risk-reduction education and information about sexual health and STI prevention.
Distribute prevention supplies including condoms, lubricants, and educational materials.
Maintain accurate and timely documentation of all client encounters and services provided.
Work with Data Specialist to ensure services client data are entered into required program data systems (e.g. AIRS and/or other agency systems) in accordance with program requirements.
Track client referrals, follow-up activities, and service outcomes.
Participate in quality improvement activities and data reporting requirements.
Participate in team meetings, case conferences, and supervision sessions.
Work collaboratively with health educators, peers, case managers, and other program staff to ensure coordinated services.
Participate in training and professional development activities related to HIV prevention and health navigation.
