About The Position

Conduent is seeking a Client Monitoring Field Specialist to support participants across the assigned region in Louisiana. This role involves daily travel and in-person visits to monitor services and ensure program requirements are met. The specialist will conduct approximately 10 in-person visits per day, update client records, and report any participant changes or concerns. This position is ideal for an independent, organized individual who excels at time management.

Requirements

• High school diploma or GED
• Available to work Monday–Friday, 8:00 AM – 5:00 PM
• Able to travel 75% daily within the assigned region, and work from home 25%
• Valid Louisiana driver’s license and auto insurance
• Basic Microsoft Office skills (Outlook, Teams, Word)
• Strong communication and organizational skills
• Ability to work independently and manage multiple tasks
• MUST Pass pre-employment background check and drug testing

Nice To Haves

• At least one year of experience working with older adults or individuals with disabilities
• Knowledge of healthcare or Medicaid programs

Responsibilities

• Travel daily within your assigned region (75% travel).
• Complete in-person monitoring visits.
• Enter information into the client database.
• Document participant updates, issues, and outcomes.
• Complete Elder and Adult Protective forms when needed.
• Follow required timelines and procedures.
