About The Position

Right at Home is seeking a Client Care Liaison for their San Luis Obispo team. This role is focused on building relationships, solving problems, and ensuring client needs are met. The ideal candidate will connect with clients and families, support caregivers, manage care needs, and uphold the agency's commitment to quality care. The work environment is fast-paced, with changing priorities, and requires organization, strong communication, good judgment, and a solution-focused approach. The Client Care Liaison will oversee the client experience from the beginning of care throughout the client relationship, working with clients, families, caregivers, office staff, referral partners, and other professionals.

Requirements

  • High school diploma or equivalent required.
  • Minimum of two years of professional work experience.
  • Valid California driver’s license.
  • Reliable, insured personal vehicle and ability to travel throughout San Luis Obispo County for client visits and other business needs.
  • Ability to meet applicable health requirements for employees who have direct contact with clients.
  • Ability to read, write, speak, and understand English as required to perform the responsibilities of the position.
  • Availability for occasional on-call responsibilities outside of normal office hours.

Nice To Haves

  • Previous experience in home care, healthcare, senior care, case management, or a medical office environment is strongly preferred.
  • Compassionate, personable, and comfortable working directly with seniors, adults with care needs, and their families.
  • Strong communication and relationship-building skills.
  • Highly organized and able to manage multiple clients, priorities, and deadlines.
  • Sound judgment and comfortable making decisions and solving problems.
  • Ability to handle difficult conversations professionally and respectfully.
  • Calm and positive in a fast-paced environment.
  • Comfortable working both in the office and out in the community.
  • Basic computer and office skills and comfortable learning new systems.

Responsibilities

  • Conduct client assessments and help develop, implement, and update individualized care plans.
  • Maintain regular communication with clients and families to ensure services continue to meet their needs and expectations.
  • Monitor the quality and appropriateness of care being provided and address concerns when they arise.
  • Identify changes in a client’s condition, needs, or circumstances and communicate those changes to the appropriate individuals.
  • Help ensure caregivers are appropriately matched and assigned based on each client’s individual needs.
  • Support and mentor caregivers in delivering safe, compassionate, high-quality care.
  • Investigate client concerns, service issues, incidents, policy violations, and other reportable events, and assist with appropriate follow-up and resolution.
  • Maintain accurate and timely client records and documentation.
  • Help clients and families identify community resources that may support continuity of care.
  • Build positive relationships with referral partners and other professionals throughout the community.
  • Represent Right at Home professionally at community events, meetings, and programs as needed.
  • Work collaboratively with our office team to solve problems and create the best possible experience for our clients and caregivers.
