About The Position

Mental Health Kokua (MHK) is seeking a CIS Homeless Outreach Case Manager to join their Oahu team. This role involves delivering outreach and housing transition services, connecting clients with essential community resources, and serving as an advocate for stability and recovery. The Case Manager will build relationships with clients in the field, coordinate care with partner agencies, and utilize problem-solving skills to help individuals overcome barriers and achieve their goals.

Requirements

  • High school diploma and three years of work experience in human services.
  • Demonstrate strong interpersonal, communication, and problem-solving abilities.
  • Be tolerant and effective in crisis situations.
  • Have the flexibility to work non-traditional hours as needed.
  • Current CPR, First Aid, and TB Clearance.
  • Valid Hawaii driver’s license, vehicle insurance, safety inspection, and a clean driving abstract.
  • Ability to pass a criminal background check.
  • Able to lift up to 25 lbs.
  • Able to travel throughout Oahu to engage clients in the field.

Nice To Haves

  • Bachelor’s degree with coursework in psychology or social work.
  • Experience supporting individuals with co-occurring mental health and substance use challenges.

Responsibilities

  • Maintain a caseload of up to 15 clients and meet with each client at least four times per month.
  • Provide field-based outreach to engage adults with serious mental illness who are not connected to care.
  • Use motivational interviewing, incentives, and relationship-building to establish trust and promote participation in services.
  • Assess needs, coordinate referrals for psychiatric, medical, and housing services, and assist with obtaining identification and health plan enrollment.
  • Complete and document clinical assessments, service plans, progress notes, and data reporting in accordance with agency and funding requirements.
  • Participate in case conferencing and collaborate with the Continuum of Care, Ohana Health Plan, AMHD, and other providers to coordinate housing placement.
  • Enter data in the Homeless Management Information System (HMIS) and complete monthly reporting.
  • Confirm insurance coverage and payer authorizations regularly.
  • Maintain accurate, confidential client records and adhere to all state and federal regulations.
  • Participate in supervision, training, and team meetings to ensure high-quality, person-centered services.
  • Perform other duties as assigned by management.
