About The Position

The Chronic Care Nurse Supervisor (RN) is responsible for the daily clinical operations, administrative functions, and financial performance of the Chronic Care Management (CCM) Program. This position provides supervision and oversight for nursing and support staff, ensures compliance with CMS regulations and organizational policies, and is accountable for the strategic growth and continuous improvement of the program. Responsibilities include identifying unmet patient needs, addressing care gaps, evaluating opportunities to expand chronic and complex care management services, collaborating with interdisciplinary teams to improve quality outcomes and the patient experience, and achieving established operational, financial, and clinical performance goals.

Requirements

BSN required.
2 years of nursing experience and 6 months of Chronic Care Management experience required.
Current Colorado RN license required.
Active healthcare provider level CPR/BLS required.

Responsibilities

Manages and develops an effective staff: providing effective communication, leadership, guidance and resources.
Determines staff qualifications and competency: recruits, interviews, selects, hires, trains, orients, mentors, evaluates, coaches, counsels, disciplines, and rewards.
Establishes and monitors staff safety and regulatory compliance.
Provides day-to-day leadership and oversight of the Chronic Care Management program.
Monitors program performance, productivity, quality metrics, patient satisfaction, and financial outcomes.
Ensures compliance with CMS Chronic Care Management requirements, documentation standards, billing regulations, HIPAA, and organizational policies.
Develops, implements, and continuously improve workflows that enhance efficiency, patient access, and care coordination.
Collaborates with providers and clinic leadership to identify eligible patients and support enrollment and retention efforts.
Monitors clinical documentation for completeness, accuracy, and regulatory compliance.
Analyzes program data and prepare reports related to quality measures, productivity, patient outcomes, and operational performance.
Participates in strategic planning, budget development, staffing, and resource allocation for the CCM program.
Develops and maintains clinical policies, procedures, and standard work.
Provides education and ongoing training to staff regarding chronic disease management, motivational interviewing, patient engagement, and regulatory updates.
Partners with physicians, advanced practice providers, social work, pharmacy, and community resources to ensure coordinated patient care.
Resolves patient concerns and staff issues in a timely and professional manner.
Maintains knowledge of current CMS regulations, evidence-based chronic disease management, and industry best practices.
