About The Position

The Chronic Care Management (CCM) Supervisor is responsible for the daily oversight, leadership, and coordination of the Chronic Care Management Program. This position supervises Population Health Coordinators, CCM Specialists, CCM Navigators, and Community Health Workers (CHW) to ensure high-quality, patient-centered care, regulatory compliance, achievement of quality outcomes, and optimal reimbursement of CCM and related services. The CCM Supervisor provides clinical and operational guidance to staff, supports patient engagement initiatives, oversees care planning and documentation processes, ensures compliance with CMS and payer requirements, and serves as a liaison between care coordination staff, providers, and leadership.

Requirements

  • Licensed Practical/Vocational Nurse; Associate’s degree in nursing or higher preferred.
  • Active and unrestricted NM or Compact State License.
  • A minimum of 3 years of Clinical Health outcome experience.
  • A minimum of 1 year of Supervisory experience.
  • Exceptional communication skills.
  • Capacity to work effectively in a team structure.
  • Ability to interact respectfully with a diverse group of stakeholders of varying disciplines and educational levels.
  • Knowledge/skills in quality improvement.
  • Knowledge of medicine, public health, and health services issues.
  • Working knowledge of standard desktop computing software packages (word processing, spread sheeting, presentation software, Internet browsers, etc.).

Nice To Haves

  • Care Coordination Management experience preferred.
  • Familiarity with health care delivery in community health settings.
