About The Position

Ozarks Community Hospital (OCH), located in Gravette, Arkansas, has an opening for a CCM Specialist within our Medical Home Department. The Scheduling Coordinator reports directly to Lead CCM and Medical Home Manager. Provides direct and indirect patient care in the office/clinic setting. Provides care that meets the psychosocial, physical, and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a registered nurse and/or physician, nurse practitioner or physician assistant and clinic manager. Maintains regulatory requirements, nursing and office/clinic policies, procedures, and standards. Communicates with providers and team members about patient's clinical condition, including results of diagnostic studies and symptomatology. Oversees Chronic Care Management including enrollment, follow-up, documentation, report running and billing procedures.

Requirements

• High school diploma or equivalent.
• Successful completion of a Certified Nursing Assistant (CNA), Medical Assistant (MA), or Licensed Practical Nurse (LPN/LVN) program.
• Current BLS certification upon hire or BLS certification must be obtained during orientation period or at the next available scheduled class.
• Throughout employment, current BLS certification must be maintained by renewal during renewal period or at next available scheduled class.
