About The Position

Ozarks Community Hospital (OCH) in Gravette, AR is seeking a Chronic Care Management (CCM) Specialist to join their Medical Home department. The hospital's mission is to provide exceptional healthcare and preventive services with compassion, respect, and dignity, focusing on caring for the underserved and improving access to care. This role reports directly to the Lead CCM and Medical Home Manager and involves providing direct and indirect patient care in an office/clinic setting. The specialist will address the psychosocial, physical, and general aspects of patient care, meet patient and family communication needs, and demonstrate initiative and responsibility under the supervision of a registered nurse, physician, nurse practitioner, physician assistant, or clinic manager. They will also maintain regulatory requirements and follow nursing and office/clinic policies and procedures. Key responsibilities include overseeing Chronic Care Management processes such as enrollment, follow-up, documentation, report running, and billing procedures.

Requirements

  • High school diploma or equivalent.
  • Successful completion of a Certified Nursing Assistant (CNA), Medical Assistant (MA), or Licensed Practical Nurse (LPN/LVN) program.
  • Current BLS certification upon hire or BLS certification must be obtained during orientation period or at the next available scheduled class.
  • Throughout employment, current BLS certification must be maintained by renewal during renewal period or at next available scheduled class.

Responsibilities

  • Provides direct and indirect patient care in the office/clinic setting.
  • Meets the psychosocial, physical, and general aspects of patient care.
  • Meets the communication needs of patient and family.
  • Reflects initiative and responsibility indicative of professional expectations, under supervision.
  • Maintains regulatory requirements, nursing and office/clinic policies, procedures, and standards.
  • Communicates with providers and team members about patient's clinical condition, including results of diagnostic studies and symptomatology.
  • Oversees Chronic Care Management including enrollment, follow-up, documentation, report running and billing procedures.
