Requirements
- CCM experience required
- Experience with eClinicalWorks (eCW)
- Strong communication and organizational skills
- Detail-oriented and patient-focused
- Comfortable with phone-based patient engagement
Responsibilities
- Manage patients enrolled in CCM programs
- Conduct patient outreach and follow-up calls
- Develop and update personalized care plans
- Coordinate care with providers and caregivers
- Document all patient interactions in eClinicalWorks (eCW)
- Educate and support patients in achieving their health goals
