About The Position

The Community Health Representative (CHR) Generalist serves as a liaison between healthcare providers, community resources, and tribal members to improve access to healthcare and supportive services. Under the direction of the CHR Director, the CHR Generalist provides health education, transportation coordination, home visits, medication assistance, case management support, and community outreach while promoting wellness and disease prevention among eligible clients. The CHR Generalist works closely with healthcare providers, hospitals, Indian Health Service (IHS), tribal departments, and community organizations to help clients overcome barriers to care and improve their overall quality of life.

Requirements

  • Must have High School Diploma or Equivalent (GED)
  • Prefer at least two (2) years of experience working in a health-related field.
  • Current valid CPR/First Aid/AED card.
  • Must have a Valid Driver’s License with clean driving record.
  • Must be able to pass a background and pass drug screening.

Responsibilities

  • Provide transportation assistance for medical appointments, pharmacies, dialysis, specialty care, and other approved health-related services.
  • Make home visits within the service area to help identify health/well-being needs and refer to the appropriate health worker/agency/program.
  • Deliver medications, medical equipment, diabetic supplies, nutritional supplements, and other approved healthcare items.
  • Assist clients with coordinating future appointments, obtaining prescriptions and coordinating medication refills.
  • Chaperone client to medical appointment if needed/requested.
  • Educate clients on chronic disease prevention and management including diabetes, hypertension, heart disease, respiratory illnesses, and other health conditions.
  • Promote preventive healthcare including vaccinations, cancer screenings, wellness exams, and healthy lifestyle practices.
  • Assist with inventory management and distribution of program supplies including hygiene products, incontinence supplies, wound care supplies, and other approved items.
  • Protect client confidentiality in accordance with HIPAA and program policies.
  • Utilize the PCC (Patient Care Component) form on a daily basis for documentation.
  • Ability to work some evenings and weekends as our program is utilized for first aid during tribal events.
  • Telephone and time management skills.
  • Attend required training, staff meetings, and continuing education.
  • Maintain assigned vehicles, equipment, and supplies in a clean and safe condition.
  • Participate in health fairs, outreach events, community education activities, and wellness initiatives.
  • Other duties as assigned.
