We are seeking qualified professionals throughout the state of Tennessee to serve members in their local communities. Applicants must be Tennessee residents and willing to travel weekly within their assigned coverage area. As a CHOICES Care Coordinator, you will serve in a comprehensive case management role, partnering with members and their families to identify supports and services that help them achieve their personal goals for health, employment, independence, and community living. You will be responsible for assessing members' needs, developing person-centered care plans, coordinating services, monitoring outcomes, and advocating access to resources that promote long-term success and quality of life. You will conduct in-person face-to-face visits, assess members' physical, behavioral, functional, psychosocial, financial, and long-term care needs, and collaborate with a multidisciplinary team of clinical and support professionals to coordinate services that promote safety, independence, and well-being. This field-based position requires weekly travel to members' homes, nursing facilities, hospitals, and other community settings to assess needs, monitor services, and ensure members receive the support necessary to remain safe and successful in their communities. Success in this role requires strong case management skills, critical thinking, effective time management, exceptional communication, and the ability to work independently while balancing multiple priorities in a fast-paced environment.
