About The Position

The Children’s Waiver Program (CWP) Coordinator participates as an Access Team member and serves as the local expert for CWP Services within Oakland County. They are to work in partnership with the local Michigan Department of Health and Human Services (MDHHS) office and Core Provider Agencies (CPA) to identify eligible children and educate Community Partners on Children Waiver Services.

Requirements

  • Master’s degree in a mental health field.
  • LLP, LP, LMSW, LLMSW, LPC, LLPC, LMFT
  • One year experience working with individuals with Intellectual and Developmental Disabilities.
  • Strong interpersonal and communication skills.
  • Be able to articulate a complex service system while establishing positive rapport with individuals in need.
  • Create a welcoming environment that demonstrates empathy and non-judgment.
  • Knowledge of OCHN provider network, and how to access additional community services.
  • Demonstrated high level of proficiency with Word, Excel, database, and other relevant software programs.
  • Demonstrated high level of interpersonal skills both in direct personal interactions and on the telephone.
  • Demonstrated high level of oral and written communication skills.
  • Demonstrated ability to handle a high volume of relatively complex assignments accurately and timely.
  • Demonstrated organizational skills needed to establish office procedures and maintain files and records.
  • Knowledge and understanding of admission and discharge criteria for each treatment program and be able to determine and designate appropriate levels of care.
  • Knowledge of DSM-IV TR and DSM-V criteria.
  • Knowledge of Michigan Mental Health Code.
  • Understanding of recipient rights policies.
  • Understanding of trauma informed principles.
  • Knowledge of Children’s Waiver Program requirements.
  • Knowledge of MichiCANS.
  • Knowledge of substantial functional limitations.
  • Must have available means of transportation to and from OCHN and for required offsite meetings or site visits.
  • Must be available for meetings and events which may occur outside of standard office hours.
  • The ideal candidate must be able to complete all physical requirements of the job with or without a reasonable accommodation.

Nice To Haves

  • CAADC, CADC, CCS, CPS, CPC-R, CCDP or development plan preferred.
  • Knowledge of public mental health service delivery systems in Oakland County.
  • Bilingual in Spanish desirable

Responsibilities

  • Serve as a liaison to the Michigan Department of Health and Human Services regarding application and prescreen processes.
  • Act as primary liaison with Core Provider Agencies (CPA) on eligibility requirements for CWP and submission of required documentation.
  • Conduct eligibility screenings for individuals with Intellectual and Developmental Disabilities (IDD).
  • Complete Prescreen for individuals determined eligible for IDD services who are requesting CWP supports.
  • Track eligibility for all CWP children not invited to the Waiver, with required reviews at six‑month and annual intervals.
  • Coordinate and ensure that OCHN Provider Agency intake occurs within seven days with the individual’s chosen CPA.
  • Enter timely and accurate data entry into the OCHN Electronic Health Record (EHR)System.
  • Enter timely and accurate data entry into the Waiver Support Application (WSA) for all children enrolled in CWP.
  • Provide technical assistance, training and coaching for CPAs and community partners on CWP eligibility requirements.
  • Participate in case reviews and/or conferences within internal teams and providers related to service delivery for children enrolled in the CWP.
  • Attends all MDHHS CWP webinar and technical assistance calls.
  • Collaborate with the Due Process Team on CWP related due process activities, including local appeals and Medicaid Fair Hearings.
  • Participate in assigned workgroups and committees.
  • Additional duties as assigned.
