About The Position

Caritas Family Solutions is seeking a Children's Residential Licensed Practical Nurse (LPN) to support the physical, emotional, and medical well-being of youth in their residential programs. This role ensures timely and appropriate healthcare, focusing on health promotion through advocacy and education. The LPN works within a trauma-informed care team environment to help youth successfully transition through treatment. The position is full-time, non-exempt, with specific working hours and reports to the Youth Residential Nurse Manager. The starting pay is $35.70/hour.

Requirements

  • Licensed LPN in the State of Illinois
  • Minimum 1-year experience in serving youth.
  • Must display a willingness to adopt trauma informed interventions, principles, and practices as well as commitment to ongoing development of their capacity for application of trauma informed care.
  • Recognizes how trauma affects everyone involved-including clients, their families, staff, and community; and impacts behaviors.
  • Responds to trauma by actively implementing the guiding principles of trauma informed care into their daily practice.
  • Resists re-traumatization of persons served and staff providing services, by creating safe, welcoming, and supportive environments.
  • Access to reliable transportation to get to/from work.
  • Must possess a valid driver's license.

Responsibilities

  • Responds to all onsite healthcare needs of children, including illnesses, first aid, PRN medications, and overall wellbeing issues.
  • Assists with, tracks, and schedules physician appointments along with documentation for supportive transportation.
  • Assists with and ensures that all children have up-to-date medical exams, immunizations, and healthcare in compliance with state and DCFS regulations.
  • Ensures that medical information for each child in the digital and paper files is accurate and up-to-date, as well as compliant with DCFS standards of care.
  • Is trained and informed on all legal parameters for consents necessary and obtains consents for medications and procedures in compliance with DCFS and state regulatory procedures.
  • Assists with training staff in regard to the health care needs of children including but not limited to medications and adverse reactions. Trains staff with all needed medication and healthcare procedures for the residential program.
  • Participates in staff meetings, Child and Family Team meetings, and other leadership and management team meetings in order to provide medical insights and information for medications and medical needs for residents.
  • Participates in new admission referral review to assist with determination of level of care and acuity of referral as it relates to supporting medical care for a resident.
  • Ensures continuity of care among nursing team by documenting and communicating actions, incidents and patient needs.
