Reporting to the respective program supervisor, the Case Manager - CLTS serves as a point of contact for participants, customers, and/or clients performing related duties toward the objectives and goals of the division. To be successful in this role during the first year, candidates should be comfortable working closely with children with disabilities and their families, building positive and supportive relationships. Success also requires being a collaborative team member who contributes to a larger multidisciplinary team. The ideal candidate is adaptable, enjoys a mix of office and community-based work, and is confident conducting home visits as part of providing services and support. Hours: 8AM - 4:30PM Monday Through Friday. Worker maintains own schedule and schedules own Home Visits. There are possibilities of comp time with this role Remote Work Possibilities: Possible to work remotely 1 day per pay period once trained and approved by the supervisor after 6 months of employment.
