About The Position

The Care Manager provides outreach and enrollment services to individuals presumed to meet eligibility requirements of New York State’s Health Home program. The role gathers assessments and enters documentation to confirm eligibility and identify areas requiring supportive services. The Care Manager works with individuals and service providers to develop a comprehensive Plan of Health Care, identifies additional providers as needed, and coordinates ongoing participation in the plan. The Care Manager documents progress and arranges transitions between health homes or to other levels of service when eligibility changes. This is a 40-hour-per-week position, Monday through Friday, 7:30 a.m. to 4:00 p.m. Hours outside this schedule may be required.

Requirements

Bachelor’s degree and two years of experience, or a master’s degree and one year of experience.
Relevant experience serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services.
Experience coordinating and participating in team settings.
Strong written and verbal communication skills.
Ability to work effectively and knowledgeably across diverse cultural, ethnic, and racial communities.
Ability to work effectively and with focus when self-directed and beyond immediate oversight.
Ability to deliver family-driven, youth-guided services.
Computer proficiency, including word-processing skills, and ability to provide culturally competent practice.
Valid driver’s license that meets The Salvation Army insurance requirements.
Access to a vehicle for home visits.

Responsibilities

Assess, engage, and enroll clients in the health home program in accordance with NYSDOH and Lead Health Home regulations.
Complete home visits with each enrolled member at least monthly, with frequency adjusted according to client need.
Collaboratively complete an annual assessment of needs, strengths, and goals with the member or family and care teams.
Facilitate at least annual meetings with members’ care teams and work with the team to develop a comprehensive Plan of Care.
Provide core services, including care coordination, comprehensive care management, transitional care, crisis intervention, and caregiver or family support.
Coordinate with care teams, make referrals to other providers, ensure follow-up after hospital discharges and emergency department visits, and support preventive medical and dental care.
Document all case activities in an Electronic Health Record according to New York State documentation quality and timing guidelines.
Meet monthly billing standards for each enrolled member and submit a billing questionnaire within the month in which the activity occurred.
Participate in mandatory and optional trainings.
Participate in weekly supervision.
Perform other duties as assigned; duties and responsibilities may change with business needs.
