Children's Case Manager
About The Position
South Central Mental Health is currently recruiting Children's Case Managers. The role involves providing one-on-one service to consumers who have a severe emotional disturbance, with the goal of preventing hospitalization or out-of-home placement and reintegrating consumers into the community. The Specialist will actively participate in administrative and treatment planning processes, using sound judgment in ongoing assignments. They will maintain positive, professional, and supportive relationships with consumers and staff from SCMH and other agencies, ensuring confidentiality at all times and keeping other staff informed of unusual situations. Progress notes must be accurate, reflect progress toward treatment goals, and be submitted weekly.
Requirements
- Have at least a BA/BS degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substitution for one year of education.
- Must have the ability to communicate positively and effectively with others, and to work independently as well as be a productive part of a team.
- Must have a dependable vehicle and valid driver’s license.
- Must provide proof of insurance.
- Pass a criminal background and driver’s license check to the satisfaction of SCMH.
Responsibilities
- Develop continuous supportive relationships with identified clients and their families.
- Assist with targeted case management/care coordination duties as needed.
- Be in contact with designated clients and families for regularly scheduled appointments to meet the client and families needs as dictated on their plan of care. Hours vary by client need.
- Maintain direct client and family contacts in their natural environment based on needs.
- Help clients learn more effective methods to manage their mental health diagnosis.
- Follow the instructions of the clinical supervisor regarding effective intervention strategies.
- Report to team leader and client’s therapist all concerns regarding clients change in behavior or reportable issues.
- Update client’s goals, objectives, and services in the treatment plan if needed and follow-up with the therapist for their clinical oversight.
- Facilitate client autonomy by helping them build on strengths, increase community integration and learn more effective controls for emotional disturbances based on medical necessity.
- Build a relationship with community agencies that will provide a natural, supportive network for clients in their quest for mastery in various situations; liaison.
- Assume an advocacy role for clients and their families.
- When necessary or possible, help integrate clients into existing emergency service systems.
- Attend and complete training sessions as required by SCMH.
- Have a minimum of 57.5% of work hours of direct contact with clients per week.
- Perform other duties as directed by supervisor.
