About The Position

This is a professional position providing Care Coordination to youth and families in Treatment Works 1.0 (TW 1.0) under the Outpatient Services Division. The TW 1.0 program provides an intensive wraparound approach to engaging and working with youth who have been identified as displaying violence or are identified as at risk of violence. The goal of services is to ensure that youth and families are stabilized and prepared to remain in or reenter their educational environment. Services will be provided in the home, school, and community by a counselor and behavioral health case manager who will be working intensively to address the behaviors that resulted in the referral to the Treatment Works program. Services will be provided to the youth and family, and caregiver involvement is required in order for successful treatment outcomes. The Children’s Care Coordinator will be responsible for organizing care activities among different services and providers. Following the Agency for Healthcare Research and Quality (AHRQ) definition fore Care Coordination, the Children’s Care Coordinator will be responsible for “deliberately organizing consumer care activities and sharing information among all of the participants concerned with a consumer’s care to achieve safer and more effective care. This means the patient’s needs and preferences are known ahead of time and communicated at the right time to the right people, and that this information is used to provide safe, appropriate, and effective care to the patient.” As such, the Children’s Care Coordinator will organize care activities needed by youth and families referred by the Pinellas County School System. That care may include services provided by Directions for Living (DFL), as well as any additional community supports or services that would benefit the youth and family. The Children’s Care Coordinator will ensure that a proper assessment has been completed to identify the needs of the youth and family and will use this assessment to organize client care activities. This may include program and service providers at DFL, identified individuals with the Pinellas County School System, as well as any additional community supports or services that would benefit the youth and family, or any other appropriate participants concerned with the clients care. The Children’s Care Coordinator operates as a member of the TW 1.0 team, and like all member of the TW 1.0 team, will ensure the youth and family have an individualized treatment plan that is strength based and developed by consensus with each member of the TW 1.0 Integrated Decision Team (IDT). All IDT’s include the client, treatment professionals, and natural supports that care about and know the youth and family best. The Children’s Care Coordinator ensures services are delivered using standards of trauma informed care principles, and that plans and clinical interventions address issues regarding how trauma impacts family functioning. The Children’s Care Coordinator ensures that services are Culturally & Linguistically Competent and are tailored to the unique culture of the child and family. The Children’s Care Coordinator will ensure that services occur in the community at a place convenient and comfortable for the family. This is a field position. The Care Coordinator is responsible for meeting an annual client facing client care number of 1,410 hours. Achieving the minimum required weekly, monthly and annual client facing client care hours is an essential function of this positon Directions for Living is an essential services provider. All Directions for Living Team Members must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation.

Requirements

Must hold at least a minimum of a Bachelor’s Degree in Psychology, Social Work or related area of study from an accredited university and a minimum of one year of previous experience working with adults and children with serious persistent mental illness.
Completion of Mental Health Care Coordinator Certification required.
Must have excellent writing, communication, computer, organizational and time management skills with the ability to work independently and as part of a team.
Proficiency in completing progress notes, treatment plans, and pertinent information concerning the consumer required.
Must hold at least a Bachelor's Degree Social Work or related area of study from an accredited university; or a Master's Degree in Social Work or related area of study.
With a minimum of one year Case Manager experience.

Nice To Haves

Master’s degree in Psychology, Social Work or related area of study preferred.
Experience with child welfare, early childhood education or children's mental health preferred.
Must become certified as a Wraparound Facilitator in high fidelity Wraparound within 3 months of employment.
Preference will be given to individuals certified as a Wraparound Facilitator in high fidelity Wraparound.
