About The Position

Morrison Center is seeking a Children's Care Coordinator in Brewer, Maine. This Care Coordinator will help to manage the clients' appointments, education support, and transportation needs, as well as coordinate and facilitate team meetings. The Care Coordinator will also attend home visits, doctors' appointments and family team meetings. Morrison Center is a private, non-profit organization whose mission is to provide comprehensive, individualized support services to children and adults with developmental disabilities. Requirements

• Bachelors degree in any field
• Exceptional communication skills
• Strong organizational and writing skills
• Ability to multitask, prioritize, and problem solve in a fast-paced environment
• Proficiency in Microsoft Office Suite

Nice To Haves

• 1+ year in Children's Services or 2+ years in social services
• Case Management experience

Responsibilities

• Manage 25-30 clients' appointments.
• Provide education support.
• Manage transportation needs.
• Coordinate and facilitate team meetings.
• Write comprehensive assessments, quarterly reviews, and continued stay reviews.
• Attend home visits, doctor appointments, and family team meetings.
• Research, coordinate, and communicate with team members.
