About The Position

Helio Health's In Community Program (formerly Circare) supports individuals and their families who encounter barriers to their health and wellness, both physical and behavioral, to develop the resources to live a satisfying and naturally interdependent life. Seeking a Care Manager to work in collaboration with a multi-disciplinary team to perform functions that will assist in the care and treatment of youth and families. Qualified candidates will possess skills in assessment, service planning, and care coordination. Care Managers will meet with families in their natural setting – home, school, or community – to develop and coordinate the implementation of a plan of care that identifies the youth’s health and wellness goals in relation to mental health care, physical health care, family education, leisure, and environment.

Requirements

  • RN plus 2 years relevant experience providing services to children, youth, and families OR Bachelor’s degree plus 2 years relevant experiences providing services to children, youth, and families OR Master’s degree with 1-year relevant experience providing services to children, youth, and families
  • Must have a valid driver’s license and own transportation
  • Helio Health's insurance carrier requires that employees who drive personal vehicles for work maintain minimum auto insurance coverage limits of at least $100,000/person; $300,000/accident and that employees provide HR with proof of current minimum insurance coverage (copy of Auto Liability Insurance Declaration Page) on their first day of employment. Employees are required to provide an updated Auto Liability Insurance Declaration Page to HR by expiration date on Declaration Page.

Responsibilities

  • Provide quality direct Care Management services to children and families in home, school, and community settings.
  • Possess skills in assessment, service planning, and care coordination with children and youth.
  • Demonstrate the ability to work from a model that is family and youth driven and supports a system of care that builds on the strengths of the child and family which is critical to their success.
  • Assist youth and families in managing and planning for safety needs and creating individualized safety/support plans.
  • Complete documentation with a high level of quality and detail to meet and exceed Agency, Health Home, and Office of Mental Health and/or Department of Health requirements.
  • Demonstrate, promote, and practice cultural competency towards clients through respect and understanding.
  • Coordinate existing services relevant to the POC, identifying and securing additional services/supports as appropriate.
  • Convene and conduct team meetings to review progress, update POCs, and confirm continuing client eligibility.
  • Thrive working independently and on a team.
  • Provide on-call availability to program participants as assigned.
  • Participate actively in trainings, meetings, and functions at the agency and in the community.
