Ongoing Child Protection Case Management provides intensive social worker services to families where abuse or neglect has been reported. Responsibilities include collaborating with families to develop case plans and safety plans to address maltreatment, conduct home visits, and coordinate comprehensive resources to ensure child safety and family stability. Case Managers monitor progress, advocate for children and families, engage networks, and participate in Court when necessary. They seek to enhance protective factors to strengthen families with the lasting goals to promote child safety, permanency, and well-being. The mission of Sherburne County Health and Human Services (SCHHS) is to promote and protect the health, safety, well-being, and self-sufficiency of Sherburne County residents. This position is located at the Government Center in Elk River, MN, however will also require work in the field.
