About The Position

This position offers a unique opportunity to be a steady, compassionate presence—someone who listens, advocates, and helps guide families toward healing and hope. Ongoing Child Protection Case Management provides intensive social worker services to families where abuse or neglect has been reported. Responsibilities include collaborating with families to develop case plans and safety plans to address maltreatment, conduct home visits, and coordinate comprehensive resources to ensure child safety and family stability. Case Managers monitor progress, advocate for children and families, engage networks, and participate in Court when necessary. They seek to enhance protective factors to strengthen families with the lasting goals to promote child safety, permanency, and well-being. Requirements

Bachelor's degree in Social Work or closely related field.
Must have a valid MN driver's license and reliable vehicle.
Social Worker I: Minimum of completed Social Work internship or closely related field/less than 1 year of Social Work experience.
Social Worker II: Minimum of 1 year of Social Work experience or equivalent education plus work experience

Responsibilities

Develop, implement, and monitor safety and case plans tailored to address and correct conditions of maltreatment and child safety, permanency and well-being in compliance with Minnesota Statutes and agency policy.
Conduct in-home and field visits to evaluate living conditions, safety, and family progress.
Assess for child safety, family strengths and needs, risk, and protective factors.
Connect families with community resources like mental health services, chemical health treatment, and housing assistance.
Determine appropriate placement or arrangements to ensure the safety and well-being of children.
Provide support for reunification and concurrent planning to promote timely permanency.
Provide resources, services, and support for youth in extended foster care.
Work as part of a multi-disciplinary team with schools, law enforcement, legal professionals, county and state agencies, tribes, and other community partners.
Prepare detailed documentation, testify in court, and implement judicial findings and court orders.
Maintain accurate, up to date case records regarding client progress and legal actions.
Prepare and submit detailed case documentation, including case notes, Court reports, and assessments in a timely and professional manner.
