About The Position

Under general supervision, this role preps, scans, and validates all types of paper documents into an optical scanning system to obtain chart completion. The position involves scanning and validating all loose reports sent to the department, analyzing medical records for deficiencies in chart documentation, and handling release of information for continuity of care requests. The role ensures adherence to HIPAA guidelines to minimize hospital liability in releasing records or protected health information. Responsibilities also include processing requests for government audits and specialized chart reviews (UVMC only), monitoring incomplete charts and following up with physicians, and adhering to guidelines from JC, Conditions of Participation, and all federal and state regulations. The position also requires performing other duties as assigned and needed.

Requirements

  • High School completion / GED
  • Medical Terminology (completion of a course or currently enrolled)
  • 1 - 3 years of job-related experience
  • Prior job title or occupational experience: Medical Office
  • Ability to read and write in order to review and maintain patient medical record charts, and perform basic mathematic calculations as well as ability to operate standard office equipment as a level normally acquired through completion of high school.
  • Must be able to work independently in a medical office setting.
  • Problem-solving abilities and basic analytical skills necessary.
  • Must possess excellent organizational skills.
  • Ability to type accurately at acceptable levels of speed.
  • Strong verbal and written communication skills.
  • Interpersonal skills required to effectively communicate, understand, interact, and respond to patients, physicians, and other personnel when obtaining and supplying routine factual medical record information via telephone or direct personal contacts on a daily basis.
  • Basic analytical skills necessary in order to determine work priorities, assist in problem resolution and recommend methods of improving workflow.
