About The Position

The Charge Nurse is responsible for coordinating and supervising assigned nursing personnel and clinical suites. This position is required to perform routine/standardized duties to the discharge of competent nursing staff management. The Charge Nurse is responsible for coordinating the assigned nursing staff according to the philosophy, objectives and policies/procedures of New Horizon Family Health Services, Inc. (NHFHS). The Charge Nurse responsibilities also include assigned daily duties of direct practitioner support and clinical patient care.

The Ryan White Program is a federally funded program that helps coordinate comprehensive medical care for people living with HIV/AIDS. The Ryan White HIV/AIDS Treatment Modernization Act of 2006 is named after a 15 year-old boy who experienced great discrimination because of his HIV status. The program is an early intervention service program designed to help improve the quality and availability of primary healthcare for those with HIV/AIDS.
Nationally, the Ryan White HIV/AIDS Program works with cities, states, and local community-based organizations to provide primary medical care and essential support service to more than half a million people each year. People living with HIV who have no insurance or are under insured can receive the HIV medical care and other support services they need.
The Ryan White HIV/AIDS Program provides funding to assist with the cost of providing outpatient medical care and related services for people living with HIV disease.
Any individual with a documented diagnosis of HIV or AIDS living within our 10-county service area in South Carolina of Abbeville, Anderson, Edgefield, Greenville, Greenwood, Laurens, McCormick, Oconee, Pickens and Saluda is eligible to participate in the Ryan White Program.

Requirements

• LPN with a valid South Carolina Board of Nursing License
• Must be able to work well with the Practice / Program Manager, Medical Staff, Nursing Service personnel and all other departments within the agency
• Must demonstrate the ability to exercise sound judgment and discretion.
• Must have a working knowledge of computer programs and professional construction of emails
• Must demonstrate: legible handwriting, excellent spelling, speaking clearly and grammatically correct, the ability to accept responsibility as indicated, being a motivated and enthusiastic learner, a courteous and tactful manner, the ability to work under stress and work well with others, following NHFHS policies and procedures regarding personal visits, cell phone usage and clocking-in on time according to your assigned shift

Nice To Haves

• minimum of 3 years’ experience preferred
• Experience to include at least two years of community nursing
• Possess strong leadership abilities in addition to basic (practical) knowledge of nursing

Responsibilities

• Collects and documents demographic data and history from the patient, family and others.
• Identifies gross manifestations of common diseases and abnormal conditions.
• Identifies gross states of faulty emotional and mental health adjustments.
• Obtains and documents vital signs and immediately reports abnormal / emergent results to the practitioner.
• Supports practitioners in the delivery of medical care, productivity and patient flow management.
• As a member of the Care Team, the supervising charge nurse will consult with practitioners and other health care team members to coordinate the provision of care and patient education in conjunction with other services / care plan activities.
• Evaluates the assigned nursing staff and completes job performance reviews as required.
• Collects information and prepares necessary reports and performance documentation.
• Works with the Practice Manager to maintain appropriate clinical staffing coverage.
• Orders supplies and other resources as required for execution of nursing tasks.
• Works with the NCSSD to coordinate Nursing, Medical Assistant and Patient Care Technician Orientation to promote competency and quality in the discharge of nursing duties.
• Implements NHFHS Nursing / Clinical Department policies and procedures.
• Works continually to assure delivery of quality, safe, efficient and competent nursing care.
• Assists in contributing input / information into the nursing personnel budget.
• Contributes suggestions, ideas and information and works with the NCSSD to refine nursing policies and procedures.
• Participates in CQI, other internal committees and special activities that promote improvement of patient flow in the clinical area.
• Maintains good public (working) relations throughout the community
• Assists, as assigned, with special projects involving community partners and the nursing staff
• Must hold all patient Protected Health Information (PHI) other patient personal information and agency information in confidence, in accordance with the attached Employee Confidentiality Statement.
• Actively participate in and comply with all aspects of the NHFHS Corporate Compliance Program, follow the Program Code of Conduct and obey all relevant laws, statutes, regulations and requirements applicable to Medicaid, Medicare and other State and Federal healthcare programs.
• Participates in CQI, other internal committees, special projects/observances or activities that promote improvements in organizational performance and/or advance the mission, goals and objectives of New Horizon Family Health Services.
• Observes schedule for work, lunch and breaks
• Performs other duties as assigned by the NCSSD, Chief Medical Officer and the Chief Executive Officer
