The Charge Nurse ensures nursing practice is founded on the ANA scope and standards, promoting high-quality care, research, and evidence-based practice. This role fosters collaboration within a transdisciplinary healthcare team and contributes to the professional development of unit staff in an Open Governance model. The Charge Nurse is accountable for effective clinical and personnel management of the unit during their shift. Specifically, the intrathecal infusion RN manages patients with implanted intrathecal pump (ITP) devices through assessments, interventions, pump refills, dose adjustments, and education. This RN demonstrates professional leadership, working under physician supervision in a clinic setting, adapting care to physician and patient goals. They are accountable for coordinating nursing care, including direct patient care, education, and transitions of care, supporting professional nursing practice across care settings. The nurse functions within the nursing care model, planning, implementing, evaluating, and communicating nursing care. Leadership is demonstrated through preceptor roles, informal leadership, and quality improvement efforts. Patient care is delegated based on skill, experience, acuity, fiscal accountability, and resources. The Charge Nurse communicates departmental issues and staff concerns to leadership and serves as a staff resource. They collaborate with the interdisciplinary team, performing duties consistent with Shepherd Center standards, policies, and procedures, including safety techniques and reporting of incidents. Participation in center-wide nursing practice initiatives and patient/family-centered principles is expected. The role involves monitoring and ensuring compliance with regulatory requirements and organizational standards, identifying risks, and supporting problem resolution. Other duties as assigned include telephonic support and other departmental nursing needs. The nurse acts as a resource for patients, caregivers, staff, and external case managers. They collaborate with physicians/APPs, patients, and caregivers to ensure intrathecal therapy orders are appropriate. The nurse performs duties consistent with ANA and State Board of Nursing standards, completing assigned training and competencies. They promote integrative care for symptom management, maintain patient records, and update EHR and device portals. Close monitoring of outpatients receiving intrathecal therapy, including refills, interrogations, and dosage adjustments, is required. Monthly rounding across all areas to confirm interrogator uploads is also part of the role. Education is tailored to age and cultural competence, promoting informed decisions and integrative care. A strong clinical background is needed for mastery of protocols and urgent interventions. The nurse must quickly assess and react to changes in patient condition, signs of overdose or withdrawal, and recognize secondary complications of ITP therapy. BLS and ACLS certification are required.
