About The Position

Mass General Brigham Home Care is seeking a part-time Charge RN to join their "Weekends" team. This role offers the opportunity to lead with skill and heart in patients' homes, providing clinical expertise, autonomy, and compassion. The Weekend RN Charge Nurse will provide clinical leadership and support to field staff, serve as a key resource for patient care coordination and clinical decision-making, and collaborate with various healthcare professionals. The position aims to ensure high-quality patient care, timely follow-up, and continuity of services, while supporting patients with complex medical needs and assisting with triage and problem-solving to prevent hospitalizations. Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home, including skilled nursing care, physical therapy, occupational therapy, speech language pathology, medical social services, and medical supplies. The ideal candidate is an experienced and emotionally intelligent Registered Nurse who thrives in a leadership role, excels in fast-paced, collaborative environments, and is confident in their clinical judgment, organized in patient care coordination, and skilled at supporting both patients and clinical teams.

Requirements

  • Current RN license in Massachusetts
  • Bachelor of Science in Nursing
  • 3-5 years progressively responsible nursing experience
  • At least 2 years in community health
  • Prior supervisory or management experience required
  • Travel required within Mass General Brigham Home Care geographic area
  • Computer literate with working knowledge of Microsoft office suite and ability to learn other software
  • Excellent interpersonal skills with ability to interact with a varied population
  • Effective written, verbal, and electronic communication skills
  • Ability to work independently, be self-directed and adapt to unpredictable circumstances
  • Ability to see a situation, critically assess it, make a decision, and execute it

Responsibilities

  • Provide clinical leadership and support to field staff throughout the weekend
  • Serve as a key resource for patient care coordination and clinical decision-making
  • Collaborate closely with Case Managers, clinicians, physicians, and interdisciplinary teams
  • Help ensure high-quality patient care, timely follow-up, and continuity of services
  • Support patients with complex medical needs recovering from surgery, illness, or chronic disease exacerbations
  • Assist with triage, problem-solving, and escalations to help prevent unnecessary hospitalizations
  • Manage staff assignments, caseloads, and productivity
  • Provide quality, clinical services to patients in various settings in the community
  • Perform admissions process and case management responsibilities in accordance with MGB Healthcare at Home policies and standards of practice
  • Utilize thorough and timely electronic documentation and interdisciplinary communications
  • Provide skilled nursing care in the form of assessment, teaching, treatment, documentation and care coordination
