About The Position

Mass General Brigham Home Care is seeking a part-time Charge RN to join the “Weekends” team. This role offers clinical leadership and support to field staff, serving as a key resource for patient care coordination and clinical decision-making. The Charge RN will collaborate with Case Managers, clinicians, physicians, and interdisciplinary teams to ensure high-quality patient care, timely follow-up, and continuity of services. This position supports patients with complex medical needs recovering from surgery, illness, or chronic disease exacerbations, and assists with triage, problem-solving, and escalations to prevent unnecessary hospitalizations. Patient care is delivered within the organizational philosophy, policy, and standards of community health practice, with admissions and case management responsibilities performed in accordance with MGB Healthcare at Home policies. Skilled nursing care includes assessment, teaching, treatment, documentation, and care coordination, demonstrating high-quality customer service and financial awareness.

Requirements

  • Current RN license in Massachusetts
  • Bachelor of Science in Nursing
  • 3-5 years progressively responsible nursing experience with at least 2 years in community health
  • Prior supervisory or management experience required
  • Travel required within Mass General Brigham Home Care geographic area
  • Computer literate with working knowledge of Microsoft office suite and ability to learn other software
  • Excellent interpersonal skills with ability to interact with a varied population
  • Effective written, verbal, and electronic communication skills
  • Ability to work independently, be self-directed and adapt to unpredictable circumstances
  • See a situation, critically assess it, make a decision, and execute it

Responsibilities

  • Provide clinical leadership and support to field staff throughout the weekend
  • Serve as a key resource for patient care coordination and clinical decision-making

  • Provide quality, clinical services to patients in various settings in the community

  • Utilize thorough and timely electronic documentation and interdisciplinary communications
  • Provide skilled nursing care in the form of assessment, teaching, treatment, documentation and care coordination
  • Demonstrate high quality customer service, and financial awareness
