About The Position

Unity Hospice is seeking a compassionate and mission-driven Chaplain to provide spiritual care and support to patients and families during their end-of-life journey. This role is an integral part of the interdisciplinary team, ensuring that spiritual needs are thoughtfully addressed. The position is 0.8 FTE, working 32 hours per week, typically 9:00 a.m. to 5:00 p.m., with one flexible weekday off. As a Float Chaplain, this role provides coverage for palliative care patients, chaplain PTO, and FMLA leaves, supports interdisciplinary team coverage, and assists with patient visits when caseloads increase to ensure continuity of care. The position participates in an on-call rotation approximately one full weekend every 6-7 weeks and works one holiday approximately every 1.5 years. Travel throughout Unity Hospice's full service area of 15 counties is required.

Requirements

Bachelor's degree from an accredited school of theology, religious studies, or related studies and/or certification in hospital or pastoral ministry required.
Valid Wisconsin Driver’s License, reliable transportation and proof of current automobile insurance coverage

Nice To Haves

Master's degree in theology, divinity, or pastoral studies preferred.
Board Certification by the NACC (National Association of Catholic Chaplains or APC (Association of Professional Chaplains) or working toward certification preferred.
Two units of Clinical Pastoral Education are preferred.
Experience in hospice and palliative care and an understanding of hospice and palliative philosophy is preferred.

Responsibilities

Provide spiritual counseling and support to patients and families as requested
Complete spiritual assessments and develop individualized spiritual care plans
Coordinate with patients’ clergy and facilitate spiritual services, prayer, or sacraments
Assist families with funeral planning when requested
Collaborate with Volunteer Services to connect spiritual care volunteers with patients
Serve as a liaison to the local religious community and provide education on hospice care
Partner with the bereavement team on memorial services and programs
Participate in interdisciplinary team (IDT) meetings and contribute to care planning
Support process improvement initiatives related to spiritual care services
