About The Position

The Chaplain plays a vital role in providing spiritual, emotional, and psychological support to patients, their families, and healthcare staff within a health care services environment. This position focuses on delivering compassionate pastoral care tailored to individuals facing serious illness, grief, and end-of-life challenges, ensuring their spiritual needs are met with sensitivity and respect. The Chaplain collaborates closely with interdisciplinary teams to integrate holistic care approaches that honor diverse beliefs and cultural backgrounds. Additionally, the role involves developing and managing programs that support volunteer involvement and community outreach, enhancing the overall care experience. Ultimately, the Chaplain contributes to fostering a supportive environment that promotes healing, comfort, and dignity for all those served.

Requirements

  • One year of experience as a Chaplain in health care preferred
  • One or more years of Hospice experience required
  • CPR Certification
  • Valid Driver’s License
  • Valid Auto Insurance
  • HomeCare HomeBase experience preferred

Responsibilities

  • Assess hospice patients and support systems to identify spiritual needs as evidenced by POC, documentation, clinical records, and IDG conference minutes.
  • Provide support for the spiritual needs of the patient/family.
  • Attend interdisciplinary team meetings and participate in the patient care planning process.
  • Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
  • Meet mandatory continuing education requirements of the agency.
  • Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
  • Promote Agency philosophy and administrative policies to ensure quality of care.
  • Demonstrate understanding of coordination and continuity of care by collaborating with the IDG.
  • Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families.
