Provides direct support services and spiritual care to patients and their families with compassion, caring, and respect. Gathers and evaluates relevant data pertinent to the patient’s situation and/or bio-psycho-social-spiritual/religious health. Develops and implements a plan of care to promote patient well-being and continuity of care. Enters information into the patient’s medical record that is relevant to the patient’s medical, psycho-social, and spiritual/religious goals of care. Incorporates patient, family as defined by the patient and other team members when planning care and service delivery. Demonstrates an attitude that creates an atmosphere of respect for human differences, beliefs, values and the uniqueness of each person encountered. Partners with patients and families to allow for effective decisions and participation in care. Shares complete and unbiased information with patients and families, with patients' consent.
