About The Position

The Chaplain Hospice provides spiritual care to patients, families, and staff within a hospice setting. This role involves conducting spiritual assessments, offering pastoral care and emotional support, and facilitating religious rituals as requested. The chaplain collaborates with healthcare teams to address holistic patient needs and may lead chapel and memorial services. Additionally, the chaplain is responsible for providing in-service education and representing the department on committees. A commitment to ongoing learning and professional development is essential.

Requirements

  • Master's Degree of divinity or arts in theology with concentration in pastoral care
  • 2 years of relevant experience in pastoral care
  • Chaplain and Fellow certification
  • Driver's License - State of Michigan

Responsibilities

  • Conducts and records spiritual assessments of patients.
  • Provides pastoral care, counsel, emotional support, and spiritual/religious rituals to patients, families, and staff.
  • Identifies additional resources and assists with referrals.
  • Documents interventions provided.
  • Collaborates closely with departmental colleagues on shared responsibilities, referrals, charting, flexibility, patient hand-offs, pager coverage, and mutual support.
  • Collaborates with the interdisciplinary health care team in identifying holistic approaches to patient care needs and facilitating progress or adjustment to illness/injury.
  • Plans and leads chapel and memorial services.
  • Provides sacraments as needed and requested.
  • Offers appropriate referrals when requests are outside of the chaplain’s ability to provide.
  • Facilitates small groups, committee leadership, support groups, and patient/family decision-making.
  • Provides in-service education for hospital units, students, personnel, and community leaders.
  • Represents the department on assigned committees and task-forces.
  • Demonstrates commitment to ongoing learning and professional development.
  • Takes initiative in identifying and accessing learning and growth opportunities.
