About The Position

The Intake Coordinator is responsible for facilitating telephone, fax and mail activity within the Child and Family Connections, Early Intervention Program at Clearbrook. This role involves maintaining program forms and public awareness materials, completing the initial intake phase of all referrals, coordinating evaluation schedules for team evaluators, and creating new client files. Clearbrook is a non-profit social services agency dedicated to providing innovative opportunities and services to individuals with intellectual and developmental disabilities.

Requirements

High school diploma or equivalent
2 years of secretarial experience
Computer proficient
Good communication skills
Good organizational skills

Nice To Haves

Some college preferred

Responsibilities

Receive and direct incoming phone calls in a cordial, professional manner.
Receive and distribute mail to appropriate staff.
Maintain an adequate supply of forms and informational materials.
File administrative and client files.
Support database activity.
Type material as requested.
Coordinate some provider evaluation schedules.
Receive all intake calls, gather pertinent information from the caller.
Follow up on all referrals and complete the initial intake information for children referred by outside referral sources.
Mail/fax medical authorization forms to primary care physician.
Mail intake packet containing consent forms to family/guardian.
Enter all intake information into Cornerstone.
Reopen, assign, and enter information into Cornerstone for children re-referred.
Create new client files.
