About The Position
We are seeking a Certified Social Worker for Home Health in Oakland, MI. This is a contract position where you will conduct psychosocial assessments, develop and implement care plans, provide counseling and crisis intervention, coordinate community resources, assist with program applications, and participate in care team meetings. You will also be responsible for maintaining accurate documentation and ensuring compliance with Medicare, Medicaid, and agency guidelines.
Requirements
- Bachelor's or Master's degree in Social Work (MSW preferred)
- Current state Social Worker license/certification
- 1–2 years of social work experience (Home Health, Hospice, or Healthcare preferred)
- Knowledge of community resources and healthcare services
- Valid driver's license and reliable transportation
- Strong communication, counseling, and documentation skills
Nice To Haves
- Home Health or Hospice experience
- MSW degree
- Knowledge of Medicare/Medicaid regulations
