About The Position
Conduct psychosocial assessments of patients and families, develop and implement individualized care plans, provide counseling, crisis intervention, and emotional support. Coordinate community resources and support services, and assist with Medicaid, disability, and other assistance program applications. Participate in interdisciplinary care team meetings, maintain accurate and timely documentation, and ensure compliance with Medicare, Medicaid, and agency guidelines.
Requirements
- Bachelor's or Master's degree in Social Work (MSW preferred)
- Current state Social Worker license/certification
- 1–2 years of social work experience (Home Health, Hospice, or Healthcare preferred)
- Knowledge of community resources and healthcare services
- Valid driver's license and reliable transportation
- Strong communication, counseling, and documentation skills
Nice To Haves
- Home Health or Hospice experience
- MSW degree
- Knowledge of Medicare/Medicaid regulations
