The Certified Registered Nurse Practitioner (CRNP) serves as the advanced practice provider and clinical leader for Pittsburgh Mercy's Medical Respite and Mercy of Care programs. This role delivers scheduled, onsite, and transitional medical care to adults experiencing homelessness or housing instability, many of whom have serious mental illness, substance use disorders, and complex acute or chronic health conditions. Working collaboratively with program leadership and the Chief Medical Officer or designated physician, the Nurse Practitioner provides comprehensive clinical assessment and treatment, promotes medication safety, supports appropriate admissions, coordinates care across healthcare and community systems, and facilitates safe transitions from hospitals and emergency departments to community-based services. The position partners closely with the Pittsburgh Mercy Family Health Center, Operation Safety Net, behavioral health teams, hospitals, pharmacies, home health agencies, specialists, case managers, and community organizations to ensure integrated, person-centered care that improves health outcomes and supports recovery and stability.
