About The Position
EverCare Mobile Health is a dedicated mental health agency specializing in long-term and residential care. We partner with independent living, assisted living, skilled nursing, memory care, group homes, HART homes, and other residential care settings. By collaborating with these facilities, we enhance care team coordination, ensure accurate diagnoses, and implement evidence-based mental health treatments. Our mission is to improve quality of life, prolong independence, and reduce caregiver fatigue among both nursing staff and family members.
Requirements
- Peer Support Certification (from the state of Oregon or Idaho – required).
- Ability to pass an enhanced IDHW/ORHW background check (required).
- Valid driver’s license, reliable transportation, and proof of insurance, clean driving record (No DUIs) (required).
- Comfortable working in a community-based setting.
Nice To Haves
- CPR certification (preferred).
- Experience using electronic health records (preferred).
Responsibilities
- Conduct assessments, treatment plan reviews, and progress notes.
- Maintain accurate and timely documentation in compliance with payer and company standards.
- Participate in supervision and team meetings.
- Collaborate with other treatment providers to ensure comprehensive care.
- Adhere to HIPAA privacy and compliance regulations.
- Assess emergency situations and escalate concerns as necessary.
- Provide consistent support to clients, promoting personal growth and accountability.
