About The Position

River Edge is actively seeking a full-time Certified Peer Specialist Case Manager. The Certified Peer Specialist Case Manager will play a crucial role in providing support to individuals on their recovery journey. Responsibilities include leveraging personal lived experience to connect with and empower clients, assisting in the development of recovery plans, and offering guidance in navigating resources and services. This role involves collaborating with a multidisciplinary team, advocating for clients, and promoting a supportive environment. The Certified Peer Specialist Case Manager will contribute to the overall well-being of individuals by fostering a peer-supported approach to recovery. Requirements

  • A valid Peer Support certification (CPS). Both CPS-MH and CPS-AD, highly preferred.
  • 1-2 years’ experience working in a behavioral health services setting preferred.
  • Proficient knowledge of WRAP and WHAM models preferred.
  • Candidates must have a primary diagnosis of mental illness or a dual diagnosis of mental illness and addictive disease, and a strong desire to identify themselves as a person with lived experience.
  • The candidate has demonstrated experience in leadership, advocacy, or governance, and is well grounded in their recovery (one year between diagnosis and application to the training).
  • Must possess a valid Georgia Driver's License.
  • Applicant will have attended Georgia Department of Behavioral Health and Developmental Disabilities Certified Peer Specialist training and passed the Certified Peer Specialist certification process.

Responsibilities

  • Collaborate with each client to develop and strengthen their Individual and Housing support plans according to individual goals and objectives using a person-centered approach.
  • Provide clients with support in sharpening personal recovery skills, accessing community resources, medical adherence, daily living skills.
  • Facilitates or co-facilitates peer-led support groups or self-help meetings, providing group skills training.
  • Maintains accurate records of peer interactions and progress, documenting daily group notes and performing case management duties.
  • Participates in ongoing training and supervision to enhance peer support skills, collaborating with other mental health professionals to ensure coordinated care for peers.
  • Initiates or contributes to peer-led initiatives promoting mental health awareness and reducing stigma.
  • Identifies and provides intervention for potential crisis situations.
  • Monitor client service usage to ensure accurate coverage
  • Maintain appropriate case documentation and client confidentiality.
  • Other assigned duties.
