Horizon Health is seeking a certified peer recovery specialist for our opioid clinic called Foundations Health. The Peer Recovery Specialists will work with other providers to ensure implementation of the Individualized service plan (with active patient involvement) through appropriate linkages, referrals, coordination, follow-up to needed support services, and direct care services. Specific activities include: assisting patient in their community, follow-up care to assess adherence to medical recommendations, assisting patients with travel in reaching important meetings and appointment as indicated in the treatment plan, and assisting patients with managing their multiple health issues. Our outpatient opioid treatment program (OTP) fosters a culture of healing and community to help with recovery. We provide medication assisted treatment (MAT), counseling and psychiatric services, all in one center, to support patients in achieving their goals.
