Adult Community Clinical Services is a Department of Mental Health-funded service that provides evidence-based interventions within a clinically focused model. It serves individuals with mental illness or dual diagnosis who live in their own homes, apartments, or group living environments. Services help individuals move toward greater independence by building skills, achieving personal goals, and maximizing natural supports. Teams are multidisciplinary and include clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff. The Certified Peer Recovery Coach provides direct peer support to individuals and families, drawing on lived and personal experience with substance use to build trust and collaboration. The coach helps individuals develop and maintain skills in stress management, relapse prevention, self-determination, self-advocacy, decision-making, and goal planning, using recovery tools such as Wellness Recovery Action Plans.
