About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Certified Occupational Therapy Assistant (COTA) for WelbeHealth PACE in Modesto, California. This is a full-time position within the PACE Program, focusing on Geriatric Rehabilitation. The COTA will join an interdisciplinary team dedicated to helping older adults remain independent and live safely in their homes. Services will be delivered in both the PACE center and participant homes, with an emphasis on maximizing independence, safety, and activities of daily living (ADLs). The role involves collaborating with physicians, therapists, nurses, and the interdisciplinary team to implement individualized care plans, fostering meaningful, long-term patient relationships. The position offers lower patient volumes and no after-hours documentation responsibilities, contributing to a strong work-life balance.

Requirements

• Associate's Degree as an Occupational Therapy Assistant from an Accredited Program REQUIRED
• Active California Certified Occupational Therapy Assistant (COTA) License REQUIRED
• Minimum Two Years of Clinical OTA Experience with a Frail or Geriatric Population REQUIRED
• Strong Understanding of Occupational Therapy Interventions for Older Adults REQUIRED
• Excellent Communication and Team Collaboration Skills REQUIRED
• Pediatric-Only Experience Will Not Be Considered

Nice To Haves

• Experience in Outpatient, Acute Care, Inpatient Rehabilitation, or Skilled Nursing Preferred

Responsibilities

• Deliver restorative and rehabilitative occupational therapy services under the supervision of a licensed Occupational Therapist.
• Provide care in both the PACE center and participant homes with a focus on maximizing independence, safety, and activities of daily living (ADLs).
• Collaborate closely with physicians, therapists, nurses, and the interdisciplinary team to implement individualized care plans.
• Foster meaningful, long-term patient relationships instead of short-term rehabilitation episodes.
