About The Position

Santa Rosa Post Acute is seeking a compassionate and dedicated Certified Occupational Therapy Assistant (COTA) to join our exceptional rehabilitation team. Requirements

  • Current California Certified Occupational Therapy Assistant (COTA) license in good standing
  • Graduate of an accredited Occupational Therapy Assistant program
  • Skilled nursing, post-acute, rehabilitation, or geriatric experience preferred
  • New graduates with strong clinical rotations are encouraged to apply
  • Experience with Net Health, Casamba, PointClickCare, or similar EMR systems is a plus
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a collaborative healthcare environment
  • Passion for helping patients maximize independence and quality of life

Responsibilities

  • Provide skilled occupational therapy treatments under the direction of a licensed Occupational Therapist
  • Implement individualized treatment plans and physician orders
  • Assist patients in improving activities of daily living (ADLs), self-care skills, upper extremity function, balance, coordination, and overall independence
  • Monitor patient progress and communicate updates to the Occupational Therapist and interdisciplinary care team
  • Complete accurate and timely daily treatment documentation and progress notes
  • Participate in care conferences, discharge planning, and interdisciplinary team meetings
  • Educate patients, families, and caregivers regarding therapy programs and home exercise activities
  • Maintain therapy equipment and treatment areas according to facility standards
  • Support facility goals for patient outcomes, customer service, and quality care
