About The Position

This role focuses on providing one-on-one occupational therapy to patients in their homes, allowing for deeper patient engagement and a more personal approach to care. Unlike traditional settings, this position avoids crowded therapy gyms and constant interruptions, enabling therapists to build trust, celebrate patient progress, and help individuals regain independence in daily living. The position emphasizes a patient-centered approach, allowing therapists the time to deliver care that is personal, purposeful, and focused on the patient's needs.

Requirements

• Active Minnesota Certified Occupational Therapy Assistant (COTA) license.
• Excellent communication and patient education skills.
• Compassionate, patient-centered approach to care.
• Valid driver's license and reliable transportation.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Deliver occupational therapy interventions under the direction of a licensed Occupational Therapist.
• Help patients improve independence with dressing, bathing, meal preparation, mobility, and other activities of daily living.
• Reinforce therapeutic exercises, adaptive techniques, and home safety strategies.
• Educate patients and caregivers on equipment and techniques that support long-term independence.
• Communicate patient progress with the supervising Occupational Therapist and interdisciplinary care team.
• Complete timely and accurate clinical documentation.
