About The Position

Comfort Creek Nursing & Rehab Center is seeking a dedicated and compassionate Certified Occupational Therapy Assistant (COTA) to join our in-house rehabilitation team. We are currently hiring for both Full-Time and Per Diem (PRN) positions. The COTA will work under the direction and supervision of a licensed Occupational Therapist to provide high-quality rehabilitation services to residents in our skilled nursing and subacute rehabilitation setting. This role offers an opportunity to make a meaningful impact by helping residents improve their ability to perform activities of daily living, enhance functional independence, and achieve the highest possible quality of life following illness, injury, surgery, or hospitalization.

Requirements

  • Graduate of an accredited Occupational Therapy Assistant program.
  • Current Certified Occupational Therapy Assistant (COTA) license in the State of Georgia.
  • Current CPR certification or willingness to obtain upon hire.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work effectively as part of a multidisciplinary rehabilitation team.

Nice To Haves

  • Experience in skilled nursing facilities, subacute rehabilitation, long-term care, or post-acute care settings.
  • Familiarity with Medicare documentation and reimbursement guidelines.
  • Experience treating geriatric and medically complex patient populations.
  • Knowledge of adaptive equipment, environmental modifications, and functional rehabilitation techniques.

Responsibilities

  • Administer occupational therapy treatments and interventions as outlined in the Occupational Therapist's plan of care.
  • Assist residents in improving functional abilities related to activities of daily living (ADLs), instrumental activities of daily living (IADLs), cognition, upper extremity function, coordination, and safety awareness.
  • Monitor resident response to treatment and communicate progress, concerns, or changes in condition to the supervising Occupational Therapist.
  • Accurately document treatment sessions, resident performance, and progress toward established goals in accordance with facility, state, and federal regulations.
  • Collaborate with interdisciplinary team members including nursing, therapy, social services, dietary services, and physicians to optimize resident outcomes.
  • Educate residents, families, and caregivers regarding therapy programs, adaptive equipment, home safety recommendations, and strategies to maximize independence.
  • Participate in care plan meetings and discharge planning as appropriate.
  • Maintain compliance with Medicare, Medicaid, state, federal, and facility regulations.
  • Support a positive, resident-centered rehabilitation environment.
