About The Position

Folkston Park Care & Rehab is seeking a compassionate and motivated Certified Occupational Therapist Assistant (COTA) to join our rehabilitation team on a per diem basis. This position supports residents in our subacute rehabilitation program through direct patient care, implementation of treatment plans, and collaboration with interdisciplinary healthcare professionals. The ideal candidate is passionate about helping patients regain independence, improve functional abilities, and achieve their highest level of performance following illness, injury, or surgery.

Requirements

  • Graduate of an accredited Occupational Therapy Assistant program.
  • Current Georgia Certified Occupational Therapist Assistant (COTA) license in good standing.
  • Current CPR certification preferred.
  • Strong communication and interpersonal skills.
  • Ability to work independently while maintaining collaboration with the rehabilitation team.

Nice To Haves

  • Experience in skilled nursing, subacute rehabilitation, or long-term care preferred.
  • Knowledge of EMR documentation systems preferred.

Responsibilities

  • Implement occupational therapy treatment plans developed by the Occupational Therapist.
  • Provide skilled therapy services to residents in subacute rehabilitation and skilled nursing settings.
  • Monitor patient progress and communicate outcomes to the supervising Occupational Therapist.
  • Accurately document treatments, patient responses, and progress toward goals.
  • Educate patients, families, and caregivers on therapeutic techniques and adaptive strategies.
  • Assist with telehealth therapy sessions conducted within the facility.
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure coordinated patient care.
  • Maintain compliance with federal, state, facility, and professional standards.
