About The Position

Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated CERTIFIED OCCUPATIONAL THERAPIST ASSISTANT (COTA).

Requirements

• Certified Occupational Therapist Assistant in the State of Texas
• Home Health experience highly preferred
• Knowledge of Home Health regulations and general healthcare compliance
• Ability to handle physical workload.
• Good driving record.

Responsibilities

• Facilitation of rehabilitation of patients with emotional, physical or mental disabilities
• Implementation of occupational therapy care plans
• Coordination of care with entire care team
• Educating patients and family members
• Preparing timely, complete and accurate clinical notes
• Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members
