About The Position

The PACE Home Care Certified Nursing Assistant (CNA) provides direct personal care and supportive services to participants in their homes, supporting independence, safety, and quality of life. This role focuses on delivering one-on-one care in a home setting, including assistance with activities of daily living, light housekeeping, meal preparation, and observation of participant condition. Working under the supervision of a Registered Nurse (RN), the Home Care CNA plays a key role in identifying and reporting changes in participants’ physical, mental, or environmental status to ensure timely intervention and continuity of care. The position requires the ability to work independently in the field while maintaining strong communication with the Interdisciplinary Team (IDT).

Requirements

  • High school diploma or General Education Degree (GED) required.
  • Must be at least 18 years of age.
  • Current State Registered Nurse Aide (SRNA) certification through the Kentucky Board of Nursing required.
  • Completion of an approved Certified Nursing Assistant (CNA) training program required.
  • Minimum of one (1) year of experience working with frail and elderly population or must receive appropriate training upon hire.
  • Food handling permit required (to be obtained upon hire).
  • Maintains current CPR certification.
  • Ability to work independently with minimal supervision and adapt to changing participant needs and schedules.
  • Demonstrated ability to provide compassionate care and effectively communicate with the adult and elderly population.
  • Ability to read, understand, and follow written and verbal instructions, policies, and safety procedures.
  • Ability to complete accurate and timely documentation, reports, and records.
  • Ability to collaborate effectively with PACE staff and members of the Interdisciplinary Team (IDT).
  • Strong interpersonal and communication skills, with the ability to interact effectively with participants, families, and interdisciplinary team members.
  • Ability to solve routine problems using standardized and sound judgment.
  • Ability to accommodate schedule changes based on participant needs and program demands.
  • Ability to follow emergency, safety, and evacuation procedures.
  • Must be a licensed driver.

Responsibilities
