About The Position

As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Requirements

  • High school diploma or General Education Degree (GED) required.
  • Must be at least 18 years of age.
  • Current State Registered Nurse Aide (SRNA) certification through the Kentucky Board of Nursing required.
  • Completion of an approved Certified Nursing Assistant (CNA) training program required.
  • Minimum of one (1) year of experience working with frail and elderly population or must receive appropriate training upon hire.
  • Food handling permit required (to be obtained upon hire).
  • Maintains current CPR certification.
  • Ability to work independently with minimal supervision and adapt to changing participant needs and schedules.
  • Demonstrated ability to provide compassionate care and effectively communicate with the adult and elderly population.
  • Ability to read, understand, and follow written and verbal instructions, policies, and safety procedures.
  • Ability to complete accurate and timely documentation, reports, and records.
  • Ability to collaborate effectively with PACE staff and members of the Interdisciplinary Team (IDT).
  • Strong interpersonal and communication skills, with the ability to interact effectively with participants, families, and interdisciplinary team members.
  • Ability to solve routine problems using standardized procedures and sound judgment.
  • Ability to accommodate schedule changes based on participant needs and program demands.
  • Ability to follow emergency, safety, and evacuation procedures.
  • Must be a licensed driver.

Responsibilities

  • Provides direct personal care and supportive services to participants attending the PACE Center, under the supervision of a Registered Nurse (RN).
  • Assists with activities of daily living, promotes safety and independence, and supports engagement in center-based activities.
  • Observes and reports changes in participant conditions.
  • Maintains a clean and safe environment.
  • Ensures accurate documentation in compliance with regulatory requirements.
  • May also assist with participant transportation as needed.
