About The Position

Under the direction and supervision of a licensed nurse, performs basic nursing care procedures. This role involves assisting residents/patients with daily living activities, monitoring their condition, and documenting care provided. The position supports the psychosocial needs of residents and requires adherence to facility policies and infection control practices. Participation in educational sessions and maintaining up-to-date knowledge of techniques, guidelines, and regulations is also essential.

Requirements

State of Connecticut CNA Certification; or within 120 days of CNA Certification course completion.
Ability to interact professionally with coworkers.
Ability to prioritize tasks.
Function as a team member.
Ability to speak, read English in order to communicate with patients, residents and clinical staff.
Provides care and services that demonstrate behaviors and the principles of person-centered care including affording people dignity, respect and compassion, offering coordinated care, support and treatment and care that is personalized.

Nice To Haves

High School diploma or GED preferred
6 months – 1 year experience as a CNA preferred

Responsibilities

Performs basic nursing care procedures under the direction and supervision of a licensed nurse, including assisting with meals, transfers, ambulation, bathing, toileting, and grooming.
Observes and communicates pertinent information about the resident/patient's condition to appropriate facility staff.
Takes and records vital signs such as pulse, respirations, blood pressures, and weight as needed.
Documents care and observations accurately and in a timely manner on worksheets and EMR.
Maintains cooperative relationships and provides support and assistance to co-workers.
Adheres to infection control practices.
Supports the psychosocial needs of the residents as per the plan of care and profile.
Participates in educational and inservice meetings to update knowledge and meets continuing education requirements.
Uses equipment in an appropriate and safe manner and adheres to the Diligent Minimal Lift Program standards.
Follows facility policies and procedures.
Performs other duties as required.
