About The Position

Rocky Mountain Care Hospice is looking for a compassionate Certified Nursing Assistant (CNA) to provide personalized, comfort-focused care to hospice patients. Our team is committed to treating every patient with dignity, respect, and exceptional care. We provide hospice and other healthcare services with a focus on responsive care and treating every patient as a unique individual.

Requirements

• Current CNA certification in good standing
• CPR/BLS certification as required
• Valid driver's license, reliable transportation, and auto insurance

Nice To Haves

• Hospice, home health, skilled nursing, or senior care experience is a plus

Responsibilities

• Assist patients with bathing, grooming, dressing, toileting, and other activities of daily living (ADLs)
• Help with positioning, transfers, mobility, and comfort needs
• Provide compassionate, one-on-one care in patients' homes or care settings
• Observe and report changes in patient condition to the hospice nurse
• Complete required documentation accurately and timely
• Support patients and families with kindness, dignity, and respect
• Follow the patient's individualized hospice plan of care
