About The Position

Midland Care Connection is seeking a Certified Nursing Assistant (CNA) to support older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE). The PACE program aims to help seniors remain safely in their homes by providing comprehensive medical care, rehabilitation, medications, transportation, social services, and supportive therapies. The CNA will assist participants with daily activities, monitor vital signs, report health changes, and provide overall support to ensure comfort, safety, and independence in both the center and their homes. This role is crucial in helping seniors maintain independence, improve quality of life, and live with dignity in their communities.

Requirements

  • High school diploma or GED required.
  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification, or ability to obtain certification within a defined onboarding period per Midland Care policies.
  • At least one (1) year of experience providing direct care to older adults, individuals with chronic illness, or persons with functional limitations.
  • Ability to assist PACE participants with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, feeding, transfers, and ambulation.
  • Demonstrated understanding of safety practices, infection prevention, and proper body mechanics in a healthcare or adult day setting.
  • Strong communication and interpersonal skills with the ability to engage respectfully with participants, families, and team members.
  • Commitment to Midland Care Connection’s mission, values, and person-centered care philosophy.
  • Physical ability to meet the demands of the role, including lifting, bending, standing, and assisting with participant mobility.
  • Current CPR and First Aid certification, or ability to obtain upon hire.
  • Ability to pass required background checks, health screenings, and meet all PACE and regulatory requirements.

Nice To Haves

  • Previous experience working in a PACE program, adult day health center, long-term care facility, or community-based senior services.
  • Experience supporting participants with dementia, Alzheimer’s disease, or other cognitive impairments.
  • Familiarity with interdisciplinary team (IDT) models of care.
  • Training or experience in person-centered care, restorative care, or dementia-capable practices.
  • Bilingual skills that reflect the communities served by Midland Care Connection.
  • Experience with basic documentation or electronic health record (EHR) systems.
  • Demonstrated compassion, patience, reliability, and a strong desire to serve older adults in the community.

Responsibilities

  • Assist participants with daily activities such as bathing, dressing, grooming, and mobility.
  • Monitor vital signs and report any health changes to the nursing staff.
  • Assist with meal assistance.
  • Accompany participants during activities.
  • Provide overall support to ensure comfort, safety, and independence in both the center and participants’ homes.
  • Follow individualized care plans and directions from nursing, therapy, and other interdisciplinary team (IDT) members.
  • Adhere to safety practices, infection prevention, and proper body mechanics.
