About The Position

Rocky Mountain Care is seeking compassionate and dependable Certified Nursing Assistants (CNAs) to join their growing home health services team in the Rawlins and Carbon County area. This community-based role allows CNAs to build one-on-one relationships with clients in their homes, helping them remain comfortable, safe, and independent. The position requires travel throughout Carbon County to provide care where clients live. The ideal candidate takes pride in providing personal, compassionate care and wants to be part of a growing home health team.

Requirements

Certified Nursing Assistant in good standing with the state (preferred, not required)
High school diploma or equivalent
Current CPR/BLS certification
Must have no disciplinary action in effect against certification
At least one year of experience as a certified nursing assistant in long-term care, home care and or hospice (preferred, not required)
A reliable means of transportation and a valid driver's license are required

Responsibilities

Providing compassionate personal care in clients' homes
Assisting with bathing, grooming, dressing, toileting, and other activities of daily living
Assisting with mobility, transfers, and positioning
Helping with meals and hydration as appropriate
Taking and documenting vital signs and other assigned observations
Observing clients for changes in condition and communicating concerns to the appropriate member of the care team
Providing companionship and emotional support
Supporting clients in maintaining a safe and comfortable home environment
Following individualized care plans and established care procedures
Communicating professionally with clients, families, and members of the healthcare team
Maintaining client privacy, dignity, and confidentiality
Completing required documentation accurately and on time
