PRN PACE Certified Nursing Assistant
About The Position
Care Guide Partners PACE, part of Hosparus Health, supports people age 55 and older in maintaining independence and living safely at home while receiving nursing-home-level care. Under the direction of PACE nursing staff, the Certified Nursing Assistant (CNA) delivers personal care and related services to PACE participants in collaboration with the Interdisciplinary Team and in accordance with each participant’s plan of care. Participants are primarily located in the clinic or day center, but may also be served in a home, long-term care facility, assisted living facility, or hospital. The position is PRN and work is as needed.
Requirements
- State Registered Nurse Aide (SRNA) certification in Kentucky.
- At least one year of experience working with a frail or elderly population, or completion of appropriate training upon hire.
- Maintain CPR certification.
- Hold a valid driver’s license in accordance with policy and provide proof of automobile insurance.
- Knowledge of safety and infection-control requirements for healthcare facilities.
- Knowledge of medical equipment use and maintenance.
- Ability to communicate clearly and effectively.
Nice To Haves
- High school diploma.
- One year of home health aide or nursing assistant experience.
- Experience using technology-based tools, including electronic medical records.
Responsibilities
- Deliver personal care and related services to PACE participants under the direction of PACE nursing staff.
- Collaborate with the Interdisciplinary Team and follow participants’ plans of care.
- Provide care to participants primarily in the clinic or day center, and potentially in homes, long-term care facilities, assisted living facilities, or hospitals.
